O Jesus, in this last hour
do not allow
that I be separated from you.
May the sufferings
of your passion
and the sacrifice of your blood
be not lost to me.

Eternal Father,
I thank you
for all the blessings
with which you have blessed me
throughout my life.

Through the merits
of the death of Jesus Christ
may I die
in your love and grace
so that I may have the happiness
