Prostrated before you, I come to ask you, my God, for the grace of a good death. I acknowledge that I have not been faithful in all things and that I have often offended you, but I sincerely ask your forgiveness, for I love you with all the breadth of my soul and all the affections of my heart. Forgive me, Lord, and be pleased with the little good you have helped me to do. I repent of my faults, and I detest them for the sole reason of your infinite goodness. I forgive with all my heart those who may have offended me.
I believe, my God, all that you have revealed to your Church. I hope in you, and I believe in your promises and in your infinite merits. I thank you for all the graces you have given me in this life, and especially for giving me the means to prepare myself for death.
Blessed Virgin, Mother of God and men, Angels of heaven, faithful guardian of my soul, Holy Protectors who have so often presented my prayers to Our Lord Jesus Christ in my name, do not abandon me at the hour of my death. So be it.
(After having said ten Hail Marys, address this prayer to the Blessed Virgin Mary): "O Queen of Heaven, please receive this crown of the Rosary, and reserve for me a holy and good death, so that, coming out of this unhappy world, I may enjoy the blessed vision of your Son, with you and the company of all the holy and blessed. So be it.
O Mother of Perpetual Help, Refuge of sinners, Hope of the dying, deign to assist me throughout my life, and especially at the hour of my death. Provide my soul, when it must leave its body, with perfect contrition, a sincere desire to glorify the Lord and to die as a victim of his love by his adorable will.
Grant that I may receive the Sacraments with faith, so that I may expire with joy under your powerful protection. So be it.
